Central Point School District 6 (D6) parents and guardians can verify their student’s enrollment information through the Online Registration portal. This annual paper process was formerly reviewed by parents/guardians at their student’s school in August. Verification of enrollment will now be completed online each year.
Importance of Current and Accurate Information
The Student Verification of Enrollment process is very important to the Central Point School District. The district uses information you provide in this annual process for safety purposes and to support academic success. By updating your data, we can do the following: distribute important information to you, contact you or others in the event of a school emergency, respond appropriately to a medical situation involving your student, and communicate your student’s academic progress. Each year, you will need to verify enrollment information for all your students. Carefully review this information to make sure all information is complete, accurate, and current.
Medical Conditions
Many of our students have medical conditions that may affect them at school. Health Services requests that parents/guardians update medical condition information each year. A school nurse may contact you to obtain more information regarding your student’s medical condition. If your student requires medication at school, please print and complete the necessary medical forms and bring them into your students school. If you need for more information, please contact your students school.
Parent/Guardian and Emergency Contacts
The District assumes the following relationships for all legal guardians:
- Lives With
- Contact Allowed
- Educational Rights
- Mailings Allowed
- Release To
The District assumes the following relationships for all non-legal guardians, such as stepparents, same-sex partners, aunts, uncles, grandparents, etc.:
- Lives With
- Contact Allowed
- “Educational Rights” and “Mailings Allowed” will remain checked unless custodial parent(s) provide written documentation stating otherwise.
- “Has Custody” will require legal documentation to be submitted to the school.
- “Release To” will remain unchecked unless you check the “Release To” box in the Parent/Guardian AND Emergency Contacts section of this form
- “Auto Dialer” Parent/Guardian is authorizing automated calls regarding attendance, school emergencies, school communications, or other school automated announcements.
Student/Family Handbook Review
The Formal Acknowledgement of Student & Family Handbook Review is part of the enrollment verification process. After reviewing the Student/Family Handbook (SFH), you must check that you and your student have reviewed it.
Accessing Online Registration
D6 parents/guardians can log into the Online Registration account if you already have a ParentVUE account activated. Parent/guardians will need to create their ParentVUE account before registering their student and will need to obtain an activation key code and activation instructions by contacting the school.
